By calling chaverim and requesting any form of assistance, I am voluntarily, knowingly, and willingly, seeking, and hereby authorizing the services of Chaverim of Crown Heights, a non-for-profit organization. It is expressly understood that Chaverim of Crown Heights is engaging in a service at my request, and that I will hold the organization and any and all of its agents, both employees and volunteers, harmless for any damage, injury, or loss that I or any other party may sustain in connection with the services that I herein request and authorize.
In accepting the services of Chaverim of Crown Heights, I expressly waive my right to sue for any damage, injury or loss as described in this paragraph.
